What you provide
The contact form asks for your name, email address, message, and an optional phone number. It also offers a separate, optional checkbox to request campaign text messages. The updates form asks for your name and email address and records your request for campaign email updates; it does not enroll you in text messages.
Where it goes
When you submit either form, the information goes to FormSubmit, an external form-delivery service, for forwarding to ShawnPackardfor64@gmail.com. FormSubmit says it retains submissions for up to 30 days. Copies received in the campaign mailbox may remain there.
The forms submit without leaving this website and display a confirmation when FormSubmit accepts the request. If an error appears, try again or contact the campaign at the address above or 207-431-8012.
Your choices
Do not include payment-card details, government identifiers, or other sensitive information in these forms.
